INSURANCE INFORMATION

There are many ways in which dental insurance plans are designed and how reimbursement levels are determined. Your dental plan is designed to assist you with dental care costs. It may not cover the total cost of your bill. The coverage plan your employer negotiated with your insurance carrier defines your dental benefits. Please read the benefit details provided by your employer to better understand your benefits.

We follow the current year’s dental fee guide of the Federation of Dental specialists of Quebec. Your insurance plan may or may not follow this fee guide. Certain variations may include plans that allow a maximum amount for a certain procedure according to the plan’s own fee guide, the non-specialist fee guide, or to the specialists fee guide but of a different year. When looking at the percentage coverage for a specific procedure, please pay attention to the maximum amount the plan will allow for this procedure as this may differ from the fee recommended by the Federation of Dental Specialists of Quebec. In certain cases, some plans exclude certain procedures completely from their list of benefits. 

At CMEP, we recommend treatments based on a diagnosis and professional judgment, not on cost of care. We make every effort to provide you with the finest care and the most convenient financial options. Once the diagnosis is made and a specific treatment is recommended, we will provide you with an insurance pre-determination form. The details of the recommended treatment cost will be completed by our staff. We will ask you to complete the portion with your personal information and submit it to your insurance company. Your insurance company will then provide you in writing your exact coverage and benefits.

Once the treatment is completed, we will provide you with the insurance claim form. When the form is received by your insurance company, it will issue the payment of the benefits directly to the patient.

Please note that we do not accept direct payment from insurance companies. The patient is responsible for all treatment fees.
